Candidates must possess Bachelor or Physiotherapy or Bachelor of Medical Rehabilitation Therapists (Registration) Board of Nigeria.

i) Five copies of curriculum vitae
ii) Photo copies of all relevant credentials.

v) Three referee reports
vi) Current Practicing License
vii) NYSC Discharge Certificate or Exemption Certificate
