GUIDELINES FOR THE 2012/2013 SCREENING EXERCISE
Screening is scheduled to hold between Monday 5th and Wednesday 7th November, 2012 .
Candidates are advised to report at the University's Mini Campus at 9 a.m. each day.
Candidates should bring with them the original and four photocopies each of the following documents:
1. Joint Admissions and Matriculation Board Examination Result Printout;
2. OSUSTECH Post UTME Screening Result Slip;
3. 3 Copies of offer of Admission by JAMB
4. WAEC/NECO Statement Of Result Issued and duly signed by the Principal Or Certificate Issued by the ExaminationBody;
5. Attestation letter from a reputable clergyman/Imam/Lawyer/Senior Civil Servant, Traditional Ruler;
6. Medical Record of Fitness and Chest X-Ray ( From Government Owned Hospital)
7. Certificate of Birth/Sworn Declaration of Age;
8. Certificate of State/Local Government of Origin; and
9. Print and fill 3 Copies of NEW STUDENT SCREENING FORM ( FORM A ) on the University website.
10. Print and fill 3 copies of STUDENT SELF DECLARATION AGAINST CULTISM FORM ( FORM C ) on the University website.
11. Passport photographs (12 copies)
