2. Proceed to d admission office with your credential/document:

ii. Original of Local Government Area of Origin certificates
iii. 5 passport photographs.
iv. Birth Certificate or Declaration of Age
v. Receipt of purchase of form/ teller of purchase

vii. Jamb score(utme print out)
3. Obtain Authority to pay from the Admission Office
4. Proceed to any of the under-listd banks 2 pay the poly fee and obtain a bank draft in favour of Delta state Poly, Ozoro
